PROTECTION OF PATIENT HEALTH INFORMATION
Africa Health Link is required by law to maintain the privacy of patient’s health information that identifies them, called protected health information (PHI). Africa Health Link is committed to the protection of your PHI and will make reasonable efforts to ensure the confidentiality of your PHI, as required by law and we take this commitment seriously.
HOW WE PROTECT YOUR PERSONAL DATA
To protect user security, we use the strongest available browser encryption and store all our data on servers in secure facilities. All data is only accessible to top management team. Our management team are bound by strict confidentiality agreements and a breach of this agreement would result in the management termination.
While we take all reasonable precautions to ensure that user data is secure and that users are protected, there always remains the risk of harm. The Internet can be insecure at times and therefore we are unable to guarantee the security of user data beyond what is reasonably practical.
HOW AFRICA HEALTH LINK USES & DISCLOSES PHI
The following categories explain the types of uses and disclosures of PHI that Africa Health Link may take for:
- TREATMENT Africa Health Link may use or disclose PHI for treatment purposes, including disclosure to physicians, nurses, pharmacies, and other health care professionals who provide you with health care services and/or are involved in the coordination of your care, such as providing your physician with your laboratory test results
- PAYMENT – Africa Health Link may use or disclose PHI to bill and collect payment for laboratory services. For example, Africa Health Link may provide PHI to your health plan to receive payment for the health care services provided to you.
- HEALTH CARE OPERATIONS - Africa Health Link may use or disclose PHI for health care operations purposes. These uses and disclosures are necessary, for example, to evaluate the quality of our laboratory testing, accuracy of results, accreditation functions and for Africa Health Link operations. Africa Health Link may disclose PHI to other health care providers that are involved in your care for their health care operations.
- INDIVIDUALS INVOLVED IN YOUR CARE OR PAYMENT FOR YOUR CARE Africa Health Link may disclose PHI to a person who is involved in your care or helps pay for your care, such as a family member or friend. We also may notify your family about your location or general condition.
- AS REQUIRED BY LAW - Africa Health Link must disclose your PHI if required to do so by law
- PUBLIC HEALTH - Africa Health Link may disclose PHI for public health activities. These activities generally include: 1) disclosures to a public health authority to report, prevent or control disease, injury, or disability; 2) disclosures to notify a person who may have been exposed to a disease or may be at risk for contracting or spreading a disease or condition.
NOTIFICATION
Africa Health Link will provide patient notification if it discovers a breach of PHI. You will be notified without unreasonable delay and no later than 60 days after discovery of the breach. Such notification will include information about what happened and what can be done to mitigate any harm.
